Stage 4 breast cancer is also known as metastatic breast cancer. This type of breast cancer has spread beyond the breast and local lymph nodes to distant organs such as bones, liver, lungs, or brain. This stage 4 breast cancer is not considered curable, but practicing numerous treatments can control the disease, extend survival, and improve the quality of life.

In this article, we’ll discover more about stage 4 breast cancer, various breast cancer subtypes, and different types of cancer treatments used to control the symptoms in stage 4 cancer patients.

Understanding Stage 4 Breast Cancer

Metastatic breast cancer, also known as stage 4 breast cancer, is the most advanced breast cancer. This stage is when the cancer has spread through the lymph system; in simpler terms, these are a network of organs and lymph nodes throughout the body and the bloodstream. Cancer patients might experience symptoms where the cancer has spread, such as bone pain if the cancer has reached the bones. Stage 4 breast cancer is generally considered incurable.

Understanding the breast cancer subtype

The treatment options for stage 4 breast cancer mainly depend heavily on the biology of the tumor and some specific characteristics of the tumor, including.

Key factors that influence treatment decisions include:

Hormone Receptor- Positive (HR+):

These types of breast cancer rely heavily on estrogen/progesterone hormones to grow. These cancer cells have estrogen receptors (ER) and progesterone receptors (PR), which allow the hormones estrogen and progesterone to bind to them and fuel the cancer’s growth.

HER 2 - Positive

Tumors usually overexpress the HER2 protein and tend to be more aggressive but respond well to targeted therapies like trastuzumab, which improves the treatment outcomes.

Triple Negative (TNBC)

These types of breast cancers lack hormone and HER2 proteins, while this subtype has fewer treatment options, but with innovations and progress in cancer research there are some new therapies used to treat this condition.

Understanding your subtype through tumor profiling is essential to knowing before selecting the cancer treatment.

Standard systemic treatments (All subtypes)

Here are some of the systemic treatments used for stage 4 breast cancer, including:

Targeted therapies

Targeted therapies precisely attack cancer cells while sparing ones.

  • Antibody-drug conjugates (ADCs)
  • Enhertu (trastuzumab deruxtecan), used for HER2-positive and HER2-low disease, significantly improves the treatment outcomes, even as a first-line cancer therapy.
  • Trodelvy (Sacituzumab Govitecan) shows better results than traditional chemotherapy in certain TNBC patients.
  • Datroway (Datopotamab deruxtecan) offers additional treatment options for some HR+/HER2 cancers.

Why these treatments matter: Using targeted therapies can slower, delay the need for chemotherapy, and often have better side effect profiles than traditional chemotherapy.

New oral targeted drugs are also considered useful to treat stage 4 breast cancers. For example, cancer therapies that degrade mutant estrogen receptors (e.g., new SERDs like castration-resistant therapy shown to reduce the cancer progression) are also widely used.

Hormonal therapies

For HR+ breast cancer, endocrine agents either hormone receptors or reduce estrogen levels. These treatments are often used in combination with:

  • CDK 4/6 inhibitors (e.g., palbociclib, ribociclib, and abemaciclib) greatly prolong progression-free survival.
  • Oral SERDs (e.g., elacestrant) for cancers resistant to earlier hormone therapy.
  • These cancer therapies are usually gentler than traditional chemotherapy and can be very effective for HR+ metastatic disease.

Chemotherapy

Chemotherapy is an effective cancer treatment, especially for cancer patients, including:

  • Triple- negative breast cancer without any actionable markers
  • Fast growing disease requiring a rapid response

Common regimens include taxanes, anthracyclines, platinum agents (carboplatin) and newer combinations.

Immunotherapy

For TNBC tumors expressing PD-L1, checkpoint inhibitors such as pembrolizumab or atezolizumab, given with treatments like chemotherapy, can enhance the immune system’s attack on cancer cells.

Other cancer therapies

The stage 4 breast cancer is systemic, local treatments might have roles, including:

  • Radiation therapy: These are helpful for pain relief or controlling growth at specific sites, including bones and brain metastasis.
  • Surgery: These treatments are usually helpful and used to manage complications and improve the overall quality of life.

These are used in some cases, along with systemic treatments like chemotherapy, targeted therapy and immunotherapy.

Supportive medicines

Bone metastasis is considered common in stage 4 breast cancer patients. Drugs like bisphosphonates or denosumab can help to reduce fractures and pain.

Symptom management helps to control pain, nutrition requirements, and mental health support, which is considered crucial for the patients.

How to choose the right treatment path?

As we all know, there is no specific treatment used to control all stage 4 breast cancer.

The key factors may include:

  • Tumor biology (HR/HER2 status, mutations)
  • Prior cancer treatments and its responses in patients
  • Patient references and treatment goals (quality of life vs. aggressive control)
  • Potential side effects due to treatments

A multidisciplinary specialist team, including oncologists, radiaologists, and surgical oncologists, and palliative care team helps to offer the best road map.
